Purpose:

This checklist documents the condition of the ice machine and related equipment at the moment of delivery. Freight damage that is not properly documented before the driver departs is commonly denied by carriers and manufacturers.

Do NOT sign the carrier’s delivery receipt as “clear” until this form is fully completed.

☐ No cracked evaporator surfaces

☐ No crushed refrigeration lines

☐ No oil residue (indicates refrigerant leak)

☐ No loose internal components

☐ Bin interior intact

☐ Door hinges and seals intact

High-Risk Damage Indicators

(These Often Cause Hidden Failure Later)

CRITICAL FREIGHT CLAIM NOTICE

Freight claims are commonly denied when:

  • Damage is not written on the BOL
  • Photos are not taken immediately
  • The delivery receipt is signed “clear”
  • The driver is released before documentation

Once the driver leaves, liability often shifts to the receiver.
